The best evidence we have is that she was Lisa del Giocondo, wife of a merchant known to Leonardo. See the site below and the section of it on the painting that outlines the evidence, which is quite strong.
I'm an art historian, and one of my colleagues who is one of the world's foremost experts on Leonardo has no doubt that Lisa was the sitter.
Incidentally, "Mona" was not a name but rather a contraction of the word "Madonna," which at the time was the equivalent of the French title "Madame". Both of them mean "My lady".

Nobody knows.There are many theories about her. One even suggests it's Leonardo himself.
However:
The most common theory is that it is the portrait of Lisa Gherardini, a rich merchants wife. It is even named "La Gioconda" that loosely translated as "wife of (Francesco) del Giocondo."
